Robotic Colorectal Surgery

Robotic colorectal surgery is a minimally invasive approach used to treat various conditions affecting the colon and rectum. By utilizing robotic-assisted technology, this procedure allows surgeons to operate with greater precision, control, and flexibility, especially in areas that are difficult to reach, such as the pelvic region.

Robotic colorectal surgery can be used for a wide range of conditions, including colorectal cancer, diverticulitis, inflammatory bowel disease (IBD), and benign colorectal tumors.

Types of Robotic Colorectal Surgery

Robotic Right Colectomy

  • Indications: Removal of the right side of the colon, often for conditions such as colon cancer, diverticulitis, or Crohn’s disease.
  • Procedure: The right part of the colon is detached, the blood supply is sealed, and the remaining colon is reconnected to the small intestine.

Robotic Extended Right Colectomy

  • Removes the right colon along with part of the transverse colon.
  • Typically performed for tumors extending across the colon or for more advanced right-sided colon cancer.

Robotic Transverse Colectomy

  • Removes the transverse colon (the middle section).
  • Used for tumors or diseases affecting the transverse colon.

Robotic Left Hemi Colectomy

  • Removes the left side of the colon.
  •  Used for left-sided colon cancer, diverticulitis, or inflammatory diseases.

Robotic Anterior Resection

  • Removes part of the rectum and sigmoid colon.
  • Treats rectal or sigmoid colon cancer, while preserving the anus.

Robotic Low Anterior Resection (LAR)

  • Removes the lower rectum while preserving the anal sphincter.
  • Used for low rectal cancer, offering sphincter-sparing surgery.

Robotic Total Colectomy

  • Removes the entire colon.
  • Used for extensive conditions like familial adenomatous polyposis (FAP), ulcerative colitis, or Crohn’s disease affecting the whole colon.

Robotic Abdominoperineal Resection (APR)

  • Removes the rectum and anus, creating a permanent colostomy.
  • Used for low rectal cancers where the anal sphincter cannot be preserved.

Surgery Management

1. Pre-surgery Preparations

  • Patients undergo imaging tests (CT scans, MRI, or colonoscopy) to assess the condition of the colon or rectum.
  • Patients are typically required to take a bowel preparation the day before surgery to empty the colon.
  • Patients may be asked to fast and avoid certain medications before the procedure.

2. Procedure

  • Anesthesia: The surgery is performed under general anesthesia.
  • Small Incisions: The surgeon makes several small incisions in the abdomen to insert robotic arms and a camera.
  • Robotic System: The surgeon sits at a console, controlling the robotic instruments with precision. The 3D camera provides a magnified view of the area being operated on.
  • Colon/Rectum Resection: The diseased portion of the colon or rectum is carefully dissected and removed.
  • Reconnection: The remaining sections of the colon are reconnected (anastomosis), or a colostomy is created if reconnection is not possible.

3. Post- surgery care

  • Patients typically stay in the hospital for 2 to 5 days, depending on the complexity of the procedure.
  • Pain is managed with medications, though patients experience less pain compared to open surgery.
